Supervised Exchange & Parenting Time

Below is information regarding our Supervised Exchange & Parenting program.

I am the custodial parent bringing my child for supervised visitation.  What do I need to do?

Below is an information packet explaining the SEPT services and application as well as a Release of Information (ROI) form.  Please review the services and complete the application, along with the ROI. You can email the completed forms to: mwright@casa7jd.org, mail the forms to: PO Box 1708, Montrose, CO 81401, or drop the forms off at: 147 N. Townsend, Montrose | 1009 Main St., Delta.  Once we receive the forms, we will contact you to go over the process, your expectations, and answer any question you may have.  We will begin scheduling visits once the visiting parent has completed their application process.

I am the visiting parent.  What do I need to do?

Below is an information packet explaining the SEPT services and application.  You will need to complete the application in this packet.  You will also need to complete the Release of Information (ROI) and the SEPT Sliding Scale form.  Once you have completed these forms, you will need call us to schedule an appointment with an SEPT Program Manager to review the guidelines for Supervised Visitation and complete your paperwork.  Please be sure to bring the complete forms (provided below) to your appointment.  Once your appointment is complete, we will begin scheduling visitation.

My children need to visit their non-custodial parent. How do I set up a Supervised Exchange?
